AHRQ Medical Office Survey on Patient Safety Culture

In response to medical offices interested in a survey that focuses on patient safety culture in their offices, AHRQ sponsored the development of the Medical Office Survey on Patient Safety Culture. This survey is designed specifically for outpatient medical office providers and staff and asks for their opinions about the culture of patient safety and health care quality in their medical offices.

Medical offices that administered the SOPS Medical Office Survey, and have not made changes to the survey, can submit their survey data to the database. Facilities submitting survey data to the database will receive a customized feedback report displaying their own survey findings as well as aggregated, de-identified findings from all facilities submitting data. Data submission will be open from September 1 to September 21.

General data submission information is available here. If you have any questions about the SOPS Database, please email DatabasesOnSafetyCulture@westat.com.

Note: NEW FOR 2017
